Office Session Fees:
$100 per 50 minute session.

Payment can be made by cash, check, or credit card.

Insurance:
Anthem Blue Cross Blue Shield

Reimbursement:
You may check with your insurance provider to see if a reimbursement
plan is offered.  If desired, a Master Bill can be created which you can
submit to your insurance company.  With this Master Bill, many insurance
companies may reimburse a percentage of your out-of-pocket fees (20, 30,
to 50+%).  

* During the first session you will have a chance to review the Informed
Consent, a standard document which details your rights as a patient.
